Intra Urethral pellets (MUSE®)

What is MUSE®?

MUSE® (Medicated Urethral System for Erection) is a treatment for erectile dysfunction that involves inserting a small medicated pellet into the urethra (the opening at the tip of the penis). The active ingredient, alprostadil, is a vasodilator that helps increase blood flow to the penis, leading to an erection.

How Does MUSE® Work?

  • Alprostadil relaxes blood vessels, allowing more blood to flow into the penis.
  • An erection usually occurs within 5–10 minutes and lasts for 30–60 minutes.
  • Works independently of sexual stimulation.

How to Use MUSE®

  1. Urinate first – A slightly moist urethra helps with insertion.
  2. Wash your hands before handling the medication.
  3. Open the package and remove the applicator.
  4. Hold the penis upright and gently stretch it.
  5. Insert the applicator into the urethra and press the button to release the pellet.
  6. Gently roll or massage the penis for about 1 minute to help the medication absorb.
  7. Remain upright or walk around for a few minutes to enhance absorption.

Dosage and Frequency

  • MUSE® comes in different strengths (125 mcg, 250 mcg, 500 mcg, and 1000 mcg).
  • Start with the lowest effective dose as prescribed by your doctor.
  • Use a maximum of 2 doses per day, with at least 24 hours between doses.

Possible Side Effects

Most men tolerate MUSE® well, but possible side effects include:

  • Mild urethral discomfort or burning
  • Mild penile pain
  • Dizziness or low blood pressure
  • Priapism (prolonged erection lasting over 4 hours) – rare but requires emergency care
  • Urethral bleeding (minor spotting)

Who Should Avoid MUSE®?

  • Men with abnormal penile anatomy (severe curvature, scarring, strictures)
  • Men prone to priapism (e.g., those with sickle cell anemia or leukemia)
  • Men with severe cardiovascular disease who are not cleared for sexual activity
  • Those with allergies to alprostadil

Advantages of MUSE®

  • Non-invasive, needle-free alternative to injections
  • Acts quickly (within 5–10 minutes
  • Can be used in men who cannot take oral ED medications

Limitations of MUSE®

  • May be less effective than injectable treatments
  • Some men experience urethral discomfort

When to Contact Your Doctor

** Seek medical help if you experience:

  • A prolonged erection (priapism) lasting more than 4 hours
  • Severe pain, dizziness, or fainting after use
  • Signs of a urethral injury (bleeding, severe burning sensation)
